I hereby authorize Trucks365, Inc., and its payment processors, banks, and financial partners, to initiate ACH debit and/or credit entries to the bank account listed above for payment of transportation, logistics, brokerage, accessorial charges, detention, fuel surcharge, or other services provided.

This authorization includes:

Payments will be processed in accordance with Trucks365, Inc. invoices and agreed contractual terms.

9Timing & Revocation

• ACH transactions may take 1–3 business days to process.
• I understand that I may revoke this authorization at any time by providing written notice at least 5 business days prior to the next scheduled transaction.
• I understand that returned or rejected ACH transactions may result in bank fees and administrative charges, as permitted by law and contract.
